Appointment Information

Appointments at Cardiovascular Surgical Clinic are usually made on a referral-only basis. To confirm your appointment time or make changes, please contact Laurel at 479-751-9022. Clinic appointments are usually scheduled within two weeks of contact from your referring physician. For directions to the clinic, please see our Map/Travel page. Please be sure to bring a referral form from your Primary Care Physician if it is required by your insurance plan. Some plans that require referral are Arkansas Medicaid, QualChoice, Health Advantage, Tricare Prime and Blue Advantage. Please bring your insurance cards and a copy of your medication list to your appointment. To save wait time at your visit, you may print your Patient Information Sheets, fill them out and bring them with you. If your insurance plan requires a co-pay, payment is expected at the time services are rendered.

As much as we try to follow our appointment schedule, unforeseen events may occur which cause our doctors to be delayed. If given enough notice, we will call to reschedule your appointment. In an emergency situation, your wait may be longer than usual and we apologize for any inconvenience.

Insurance Plan Information

Cardiovascular Surgical Clinic accepts Medicare Assignment and participates in most major health care plans available in Arkansas. We are network providers for Blue Cross/Blue Shield, Tricare, Medicaid for the states of Arkansas, Oklahoma and Missouri, Health Partners and Premiere Care. To “participate” means we accept that particular insurance company’s allowables. This does not include your co-insurance, deductible or any service deemed patient responsibility by the rules of your specific insurance carrier or state assistance program. We will file Medicare, Medicare supplement and commercial insurance claims for you. The Business Manager or Office Manager can assist patients experiencing financial hardship in setting up a payment plan. After all insurance monies have been received, you are immediately responsible for full payment of any remaining patient balance. If full payment cannot be made, it is your responsibility to contact our office immediately to set up a monthly payment plan.

Medicare Policy

We file insurance claims as a courtesy to our patients. We will not become involved in disputes between you and your insurance company regarding deductible, covered charges, co-payments, discounts, secondary insurance, “usual and customary” charges, etc., other than to supply factual information as necessary.

Payment by VISA and MasterCard are also accepted.
